At Healthmap Solutions, our number one priority is the health of our kidney patients. We partner with patients’ providers, such as primary care physicians, nephrologists, cardiologists, and endocrinologists, to support and manage the whole patient in our Kidney Health Management program. Our team of clinical and non-clinical staff work with providers to identify, manage, monitor, and close potential gaps in care, helping patients live healthier lives.

It’s important to build strong relationships with patients as we work together to ensure the best care possible. Our Care Navigation team talks directly with patients about their individual experiences and needs. This helps us identify personalized patient support through varying components, such as resources and education, medication evaluation, diet review, and much more. Our mission is to work with patients and their providers to ensure they live healthier, longer lives.

Kidney Health Management (KHM)

What is Kidney Health Management?

Our Kidney Health Management (KHM) solution is designed to improve the health of people with chronic kidney disease and other chronic conditions, such as diabetes and high blood pressure. We can identify potential health issues, provide education, and more as we become a primary resource for patients within our KHM program.

How will I be selected for kidney health management?

You may be selected for the KHM program based on information about your unique health needs provided by your doctor or your health plan. Your doctor may specifically recommend inclusion in the program if you have health conditions that may affect your kidneys, such as diabetes or high blood pressure.

Kidney Health Management Services

Services are tailored to each individual patient’s needs. As part of the KHM program, you may also qualify for our Care Navigation services.

Here is What You Can Expect

  • A member of the Care Navigation team will personally reach out to you to introduce the KHM program and answer questions
  • We’ll review your medical history and medications, and work with you to complete a Health Risk Assessment
  • We’ll teach you to manage your conditions, medications, nutrition, and more
  • We’ll help you understand and manage instructions from your doctor such as appointments, other healthcare service needs, and procedures
  • You can get help scheduling visits with your doctors
  • We can help you deal with some common life challenges by connecting you to community-based services that can assist you, such as transportation, meals, and help with financial burdens through our Social Determinants of Health capabilities
  • We offer language assistance if you need it
  • You can call the Care Navigation team at any time to ask questions or get advice

Care Navigation

What is Care Navigation?

Care Navigation is a complex care management clinical service to support patients’ needs between doctor visits. Care Navigation is a multidisciplinary team that focuses on identifying and removing barriers that may prevent you from following your doctor’s recommended health plan.

How do I reach the Care Navigation team?

You can reach the Care Navigation team Monday through Friday, 8 a.m. to 8 p.m. Eastern Time by dialing toll‑free 1‑800‑481‑0474 (TTY: 711). If you live in Puerto Rico, dial toll-free 1-800-203-8703 (TTY: 711) Monday through Friday, 7 a.m. to 7 p.m. AST.

Can I Opt Out of Care Navigation?

While your doctor and health plan may recommend you for Care Navigation services, joining the program is voluntary. You may opt out at any time.

Rights and Responsibilities

You have the right to:

  • Be treated politely and with respect
  • Receive understandable information
  • Have information about Healthmap, including:
    • Our staff and their qualifications
    • The programs and services we offer
    • Any business relationships that may affect you
  • Be supported to make healthcare choices with your doctors
  • Be told about all treatment options even if a treatment is not covered and to discuss choices with your doctors
  • Have your medical and personal information kept private, and confidential
  • Know who has access to your information and how we keep it secure, private, and confidential
  • File a complaint and understand how and when it will be resolved
  • Know the names of your Care Navigation team and how to ask for a different Care Navigator (if you are in Care Navigation)
  • Opt out of Care Navigation at any time without penalty

You have a responsibility to:

  • Follow the plan you and your doctor (or you and your Care Navigator for those in Care Navigation) agree upon
  • Provide correct information to your doctor and your Care Navigator (if you are in Care Navigation)
  • Tell your doctor and Care Navigator if you decide to leave the program (for those in Care Navigation)
